Notary Commission Registration
To become a Notary Public in Minnesota you must apply via the Minnesota Secretary of State website.
Notary Commission Card
The Minnesota Secretary of State will issue a Notary Commission card. This card must be registered with your resident Minnesota county, in person or by mail.
County Registration
When you register with the county, please bring:
- $20 fee
- Government issued picture identification
- Notary Commission card from The Minnesota Secretary of State
By Mail
When mailing your notary, please mail:
- $20 check or money order made payable to Polk County Recorder
- Notary Commission card from The Minnesota Secretary of State
- A photo copy of your government issued picture identification
- Send to:

Name Change
The same application can be used to change your name on your Notary Commission card. The new card would need to be registered with the same county in Minnesota in which you originally registered. The $20 fee is required each time you file.
